Trooper Investigating Fatal-Single Vehicle Crash (Friday, Dec. 1) on I-495 in Haverhill


On the afternoon of Friday, Dec. 1, 2023, Massachusetts State Troopers responded to a single-vehicle crash on Interstate 495 in Haverhill that resulted in the fatality of the operator.


Preliminary investigation indicates the following. At approximately 2:54 PM on Friday a 2017 Honda CRV was southbound in the left land of I-495 south. Just prior to the Route 97 overpass, for reasons still under investigation, the CR-V exited the roadway and entered the grass median, where it struck a guardrail. The collision with the guardrail caused the CR-V to roll over onto its roof.


The operator and sole occupant of the vehicle, David Ziembko, 62, of New Britain, Conn., was extricated by first responders, who began attempting emergency life-saving measures. Mr. Ziembko was transported to Lawrence General Hospital, where he was pronounced deceased.


The facts and circumstances of the crash, including whether Mr. Ziembko may have suffered a medical event prior to driving off the road, remain under investigation by the State Police-Newbury Barracks, State Police Collision Analysis and Reconstruction Section, and State Police Crime Scene Services Section.


Troopers were assisted on scene by the Haverhill Fire Department and Trinity EMS. Two lanes of I-495 south were closed for approximately 3 hours to accommodate the rescue response and crash investigation. No further information is being released at this time.
